What to expect at your first visit:
At your initial visit, we complete a comprehensive exam to address your needs and concerns. This includes a review of your medical and dental history, an oral cancer screening, x-rays (as needed), photos, charting of your teeth and gums, a thorough examination of your oral cavity (including an oral cancer screening), joints and muscles, and a conversation about your interests and goals. We will discuss the various treatment recommendations and the alternative treatments available to you. We encourage you to ask questions, as we want you to be informed and comfortable with all aspects of your care. Our financial and insurance coordinators are also happy to review the financial aspects of the proposed treatment options.
Patients seeking a consult regarding veneers, whitening or other aesthetic concerns are encouraged to bring in photos and ideas regarding their goals and desires. This will provide us with as much information as possible to provide you with an accurate treatment plan.
Office Cancellation Policy:
If for any reason you cannot make your appointment, please provide us with at least 48 hours notice. Appointments not cancelled 24 hours in advance may be subject to a cancellation fee.
New Patient Paperwork:
Save time by filling out your new patient forms online. If you prefer to print out the forms and fill out beforehand, click on the download option for each individual form. Forms can be e-mailed or faxed to our office, or you may bring them with you to your appointment.
